Being a Reablement Health Care Worker is so much more rewarding than you think. In this role, you aren’t taking care of people in a healthcare setting – you are helping them to manage their own personal care within their own homes. By focussing on abilities and outcomes, you are assisting them to improve their long-term independence.
The role means you will be supporting individuals who have been discharged from hospital or may have had an illness, crisis, fall at home or change in circumstances in relation to themselves or their carers. Our main aim is to help individuals gain confidence by working alongside them and sharing in their pride as they achieve goals and gain independence.

Wavell Close is a Supporting Living Service consisting of two service users who live in the house as tenants. The service provision offers a personalised service to vulnerable adults, aged over 18 years who have learning disabilities and/or autism.
The successful candidate will oversee the delivery of a range of services in the Supported Living Scheme for people with learning disabilities. To work with residents, in a variety of settings to promote greater choice, skill development, independence and inclusion.
There is a requirement to work evenings, weekends and bank holidays in accordance with a planned rota.
Experience of working with vulnerable people in residential based settings is essential. A Level 2 Diploma in Health & Social Care or equivalent is also essential.

An exciting opportunity has arisen to join our Supported Living team as Assistant Service Manager. This service compromises of 4 houses within the Sefton area, providing a home with support to adults who have varying support needs, enabling people to live an independent life in their own home, as tenants in the local community.
The aim of the service is to provide a purposeful environment, where adults can have positive self-identity and make informed choices in all aspects of their lives.
You will assist with the day to day management and development of the Supported Living Service in accordance with fundamental standards of quality and safety, other relevant legislation and Company policies and procedures. Deputise in the absence of the Registered Service Manager ensuring provision of an effective and efficient Supported Living Service.
Responsibilities will include staff supervision and support, budgeting and administrative tasks, and working in partnership with family members and external agencies.
Although the post is Monday to Friday the post-holder must be flexible as it is a 24/7 service and therefore needs of the service will need to be met. The post holder will be required to undertake on-call duties as required in order to provide effective management cover.
A Level 3 Diploma in Health & Social Care or equivalent is essential. Experience of supervising staff would be advantageous.

Lyra Road is a Supporting Living Service consisting of service users who live in the house as tenants. The service provision offers a personalised service to vulnerable adults, aged over 18 years who have learning disabilities and/or autism.
The successful candidate will participate, on a rota basis, in the day to day running of the Supported Living Scheme. You will work with service users in a variety of settings to promote greater choice, skill development, independence and inclusion.
There is a requirement to work evenings, weekends and bank holidays in accordance with a planned rota. Sleep in duties are also required for which you will receive an additional payment.
Experience of working with vulnerable people in community or residential based settings is essential. A Level 2 Diploma in Health & Social Care or equivalent is also essential.
An Enhanced DBS Check is required.

An exciting opportunity has arisen to join our team at Chase Heys. The post holder will carry out a range of domestic duties to ensure a high standard of cleanliness and ensure a pleasant, safe and clean environment is maintained at all times. You will also assist in the kitchen, including serving meals and refreshments.
No specific qualifications are needed, however experience of undertaking general cleaning and domestic tasks is essential.

An exciting opportunity has arisen to join our team at James Dixon Court. Working as part of an integrated team delivering care and housing related support in what was traditionally a 30-bed residential home. Recently, we have changed the focus of the service and redesigned it into a reablement bed facility to meet local demand. The primary focus of this service is to enable people to reach their maximum potential which will increase the opportunity to remain living independently in their own home.
You will contribute to meeting the assessed care needs of service users in a way that respects the dignity of the individual and promotes independence, demonstrating a sensitive, understanding, and patient approach.
The post holder will be part of a team ensuring an environment is provided where people we support feel secure and comfortable. The successful person will support individuals with daily living tasks, reporting directly to their line manager any aspect of service user care, that requires investigation or urgent action. Complete effective, timely and appropriate records and information such as case notes, medication charts and have an awareness of confidentiality.
A Diploma in Health & Social Care Level 2 or equivalent is desirable.
